Required Documents for Companies
In order for your application to be processed as quickly as possible, please have the following documentation available for your 
subscription:
Α. Private Law Bodies Corporate
- Identity card, passport, driving license of the person who fills in the Application form - Agreement. (To be shown).
- In case of authorization provided by the company’s legal representative to a third party, the authorization should be written on the company’s letterhead, bearing the company’s seal, signed by the lawful Company representative and accompanied by an affirmation of the authenticity of the signature. (The authorization letter shall include the Tax payer’s identification number, the Public Tax Office and the address of the Company, as well as, the number of transponders requested to be activated in their subscription program). |Authorization Form|
- Seal of the Company (unless the company letterhead had been stamped).
- Detailed list of the vehicles to which transponders will be supplied (with a description of the number of axles and height). [Vehicles List Form attached]
- Public Law Bodies Corporate
- Identity card, passport, driving license of the person who fills in the Application form - Agreement. (To be shown).
- In case of a subscription taken out on behalf of a Service, the authorization letter should be signed by the Director of the Service and accompanied by an affirmation of the authenticity of the signature.
- Formal letter of the Service specifying the person authorized to sign (to be shown)
- Detailed list of the vehicles to which transponders will be supplied (with a description of the number of axles and height).[Vehicles List Form attached]
